Dropped Object on SeaRose FPSO

On September 7, 2025 Cenovus Energy reported an incident that occurred during cargo transfer operations on the SeaRose FPSO.

Following the lift of a compressed gas cylinder rack from the M18 deck, the SeaRose FPSO encountered significant vessel motion. In response, the crew opted to return the rack to the M18 deck. During the descent, the cylinder rack made contact with a crane rest, which caused the cylinder rack door to swing open. As a result, two cylinders—each weighing approximately 100 lbs— fell from the cylinder rack. One cylinder fell about 45 feet into a barricaded area below, and a second fell approximately five feet onto the M18 laydown area.

Crane operations were immediately suspended. The area below the lift had been barricaded at the time and no personnel were inside the exclusion zone. Due to the weight of the object and the distance travelled, the incident had the potential for fatality based on the Dropped Objects Prevention Scheme calculator.

Cenovus has initiated an investigation into the root cause of the incident, which the C-NLOER is monitoring.
