On June 25, 2025, Hibernia Management and Development Company Ltd. (HMDC) submitted its investigation report from an incident that occurred on the Hibernia Platform on June 11, 2025.
At approximately 10:30 a.m. on June 11, crew were installing a half tonne lighting pole that was rigged with two wire slings and guided into position using a tagline. After lowering the pole into position, a worker installed the first bolt to secure the base of the pole to the structure. As the team worked to align the remaining bolt holes, both wire slings simultaneously failed. This caused the pole to shift and rest against the upper rail of a temporary scaffold.
Two workers on the tagline helped reduce the impact of the pole against the scaffold. The previously installed bolt held the base of the pole in place, preventing it from falling further. Two workers were on the scaffold at the time of the incident. No injuries were reported.
The work area was secured, access to the scaffold was restricted and installation of remaining light poles was paused. The incident is a near miss with the potential for fatality. HMDC is implementing corrective actions, including enhanced lift planning, improved communication, and updated procedures to prevent recurrence.
